Fair Play OOSH is excited to share our Vacation Care Program. Our Vacation Care program is designed with the children at the forefront of all planning. Below is our booking form for our Vacation Care Program; please fill out accordingly, read the important information section and sign at the bottom once complete. Forms that have not been signed by a parent or guardian will not be accepted and your child/ren will not be booked in. Those parents who are yet to enrol your children into our OOSH/Vacation Care, will need to enrol online at www.fairplayoosh.com.au

*Please note that all bookings are subject to availability and filling in this form does not guarantee your spot. The service will notify you to advise that your bookings have been confirmed*

Before proceeding you must completed an enrolment form for you child/ren. Please go to www.fairplayoosh.com.au to complete an enrolment form for your child/ren,  Once that has been completed and submitted, please proceed to complete the VC booking form.

Permissions

I give permission for my child/ren to attend Fair Play OOSH and will not hold Fair Play OOSH, its staff or volunteers responsible for damages and/or loss of property or accident.

I agree to my child/ren travelling by Fair Play OOSH buses or Hire Buses/coaches where required.

I give permission for my child/ren to travel on public transport and to walk where appropriate as part of the excursion program. 

I give permission for ambulance, medical, hospital or dental assistance in an emergency and agree to pay all incurred costs.

I agree to pay my account on the due date and understand the consequences of an overdue account.

I understand that I am required to pay 2 weeks fees in advance.

I understand that due to cessation of care requirements that if my child is absent on the first day of attendance (new enrolment) or last day (if no attendance has occurred within 14 weeks) I will be liable for the full fees without subsidy.

I understand that there is no cancellation of excursions/incursions once booked & day camps require a minimum of 24hrs notice.

I understand that should my child/children be present after the service closing time, a late fee will apply of $5 per min/per child.

I understand that if my child purposely damages property, I may have to cover the cost of repair or replacement.

I understand that if my child’s behaviour is disrespectful, violent, or dangerous to other children or Educators that I may be called to collect my child from the service or excursion. Continued behaviour may also result in suspension from the service where I will still be charged.

I understand that the service will refer to the photo restrictions on my enrolment form.

I understand that if I have outstanding fees my Vacation care booking may not be accepted until my account has been paid.

I acknowledge that G & PG rated movies may be shown at the service.

I Understand that the service may engage in activities that could involve the children getting messy i.e water play, mud play, makeup, nail polish etc

I understand that valuables are not to be brought into the service (mobile phones, iPads etc) unless stipulated, and Fair Play OOSH take no responsibility for these items.

I understand that my child is allowed to bring their bike/scooter to the service when authorised by staff and I agree and understand that a helmet has to be worn at all times, Fair Play OOSH takes no responsibility for broken/lost bikes or scooters and all bikes must have working breaks

I understand that my child is allowed to bring their personal electronic device to the service when authorised by staff and I agree and understand that my child will have limited Screen (service discretion or as advise by parent/guardian). G or PG Games are only allowed to be played. My child will have no access to the internet. Fair Play OOSH takes no responsibility for lost or damaged electronics, these are the responsibility of the child.

A text will be sent out to families booked on the day before advising any requirements for the day including any travel details

Our service has children who attend with anaphylaxis. We are a NUT FREE service, please do not pack any foods containing nuts including Nutella, nut bars or peanut butter.

I am aware that this form is my authorisation for my child/ren to attend the above selected excursions.

In the event of injury, no accident or medical insurance cover is provided by Fair Play OOSH PTY LTD for children participating in excursions, incursion or service-based activities, unless there is a breach of duty of care by Fair Play OOSH Staff. Fair Play OOSH is insured to meet the financial impact of any legal liabilities arising from its activities. It does not provide, nor has it ever provided, accident or medical insurance for children enrolled in their services.

I understand that a new booking form is to be completed if i require additional days after submitted my original days and in extreme last minute cases, a text message is required to give authorisation for the excursion until the online booking form can be completed.

I agree that the information I have provided is correct, I understand the services policies and procedures for vacation care as listed within the vacation care booking form.
